Thank you for taking the time to fill out an ID card request form. Please make sure to email your photo to registrar@cmasas.org. Please note we cannot mail ID cards outside of the U.S.

ID Photo Requirements:

  • Recent color photo
  • Student facing forwards in center of photo
  • Full face and shoulders showing (80% of the photo should be head and shoulders)
  • Clear lighting
  • Plain background
  • No filters added
  • No selfies (Please have someone take your photo)
